Dental Benefits
Dental coverage is one type of benefit provided by your health & welfare
Plan. There is no preferred provider for dental
benefits, you may use any dental provider without an out-of-network
penalty.
Annual maximum benefits
for dental range between $500 to $2,000 per covered
individual depending on the Plan in which you are enrolled.

overview of covered services. For a comprehensive listing, please
review your Plan Booklet.
Orthodontic services are covered only for eligible dependent
children.
