Dental - Deductibles,
Maximums and Co-Payments
Some of the dental plans have deductibles that you
must pay before the Plan will pay at the specified
co-payment rate. Annual maximum benefits vary from $1,000
to $2,000 per covered individual. You may review a
summary of the deductible, maximums and co-payment rates
for your plan:
Once deductibles have been met, payment will equal
the co-payment rate multiplied by the actual charge (or
the usual and customary charge for that service,
whichever is lower).
Dental services are divided into three Parts:
Part I - Preventative Dentistry,
Part II - Restorative Dentistry, and
Part III - Major Dentistry.
Co-payment rates vary for Part 1, II and III.
You may review an overview of the covered services at
the
Dental - Summary of
Benefits.
