Summary Plan Description

The Summary Plan Description (SPD) explains in detail your eligibility, your benefits, and your rights under the Plan as required by ERISA.  The SPD may be affected by a plan amendment - be sure to check below for any Summary of Material Modifications.  Each participant receives a hard copy of the SPD when they enroll in the Plan, which is often referred to as your Plan Booklet.

Summary of Material Modifications

A Summary of Material Modifications is mailed to all plan participants when a material change is made to the Plan.

• Benefit Improvements Effective January 1, 1996
• Benefit Improvements Effective January 1, 2001
• Plan Update Effective January 2005
• Plan Changes Effective July 1, 2005
• Plan Change Effective June 1, 2006
• Benefit Improvements Effective January 1, 2008
