New Baby or Adoption of Child
Change in Custody
Enroll your new baby or adopted child for Medical, Dental and Vision benefits
If you have Health & Welfare coverage, you must update your
Enrollment Form. Include a copy of the birth
certificate or adoption papers when submitting the enrollment form.
You may fax in the form and birth certificate or adoption papers.
If your spouse also has coverage, you may wish to
review how to
coordinate benefits with his or
her health insurance.
Complete and
submit the Medical/Dental Annual Claim Form
Claims for your dependent child will not be processed
until the
Medical/Dental Annual
Claim form is received at the Administrative Office.

Review your beneficiary
designations
You may complete the following forms if wish to change your
beneficiary designations. Please note that the Administrative
Office
must receive the original form to change a beneficiary.
Prepare or update your will
If you have Legal coverage, now is a great time
to update or have a will prepared using your
Legal
benefits.
Complete the Step Child/Foster Child Questionnaire (if not your
natural children)
If your custody
arrangements have changed, i.e. step-children now live with you, you
will need to complete the
Step Child/Foster Child Questionnaire.
The Administrative Office will notify you if your step or foster child has been
approved as a covered dependent.
