Life Events - Relocating

If you move to a new location, please file a Change of Address form.  This will ensure receipt of essential correspondence from the Administrative Office, such as:

• Benefit payments

• Quarterly Money Purchase Pension Plan statements

• Annual Pension Plan statements

• Notification of plan changes

• Explanation of benefits for health benefits

Before receiving medical treatment, you may also wish to review the preferred providers available in your new location.
