Coverage
In the event your coverage ceased due to termination of
employment, reduction in hours, or your death, the
Administrative
Office will send a termination notice. You will have 60 days from the date of
notification to elect to continue coverage.
Effective January 1, 2018, coverage can be continued by "self-paying" for up to
36 months following the
last month you were eligible through your employer or hour
bank. As of January
1, 2001, retirees may continue their coverage under the
retiree provision if they meet the eligibility requirements.
To
"self-pay", contact the Administrative Office in order to determine
your monthly premium and length of time that you may self-pay.
After your initial self-payment, subsequent payments need to
be submitted to the Administrative Office by the first of each month
that you wish to have coverage. There is a 30 day grace
period; however, payment will not be accepted after the 30
days or the end of the month, whichever is later. Your self-pay coverage will end. Once
coverage has ended you may not resume self-pay until
eligibility has been re-established under the Plan through
your employer.
Self pay coverage
for you will cease on the date any of the following occurs:
- the Alaska Electrical
Legal Fund ceases to provide group legal coverage;
- self-pay coverage
premiums are due and unpaid;
- an individual
re-establishes eligibility under the Plan through their employer;
- at the end of 36
months of self-pay coverage.
