Summary Plan Description
The Summary Plan Description (SPD) explains in detail your
eligibility, your benefits, and your rights under the Plan as
required by ERISA. The SPD may be affected by a plan amendment - be sure to check
below for any Summary of Material
Modifications. Each participant receives a hard copy of the SPD
when they enroll in the Plan, which is often referred to as your
Plan Booklet.

change is made to the Plan.
