Events - Temporary or Permanent Disability
Temporary disability - occupational
A disability is considered occupational
if it is due to bodily injury or sickness which is covered by a Worker's
Compensation Act or similar legislation. Please see the
Summary Plan
Description for details. Only the Plan 500 has
weekly income disability benefits for an occupational disability.
To be eligible for benefits you must be:
-
Currently eligible under Plan 500 as an active participant; and
-
Continuously eligible for 24 of the 60 months
immediately preceding your disability; and
-
Incurred the disability while working
for an employer contributing to the Plan.

Temporary disability
- non-occupational
A disability is considered non-occupational if it
is not the result of any injury or illness sustained while engaged in
any occupation for wage or profit. Please see the
Summary Plan
Description for details.
To be eligible for weekly income disability benefits you must be eligible
under a Plan covering active participants (not a retiree or COBRA
plan) on the date your disability begins.

Filing for
weekly income disability benefits
To apply for
weekly disability
benefits, you
must complete the
Application for Weekly Income Disability Benefits
form. Your
physician must also complete Part 2 of this form, the “Attending
Physician’s Statement”. Mail or fax your completed
application to the Administrative Office.
Once your application is approved, you will need to complete the top half of the
Supplementary
Disability Claim Form each week for as long as your disability
continues. Once every four weeks your doctor will need to
complete the bottom half of this form, the “Statement of Attending Physician”. Mail or fax
your completed claim form each week to the Administrative
Office as
listed on the form.

Extension of
Health & Welfare Eligibility
Your eligibility under the Health &
Welfare Plan will be extended for up to six months during your
qualified temporary disability regardless of whether the disability is
occupational or non-occupational.

Permanent
disability
Pension Plan (Defined Benefit)
You may be eligible for disability
retirement through the Pension Plan if you experience a total and
permanent disability due to injury, disease, or mental disorder which
permanently prevents you from working at a gainful occupation for
which you would be expected to do in light of your training,
experience, and abilities.
To be eligible for disability retirement
you must be vested in the Pension Plan and you must have earned a year
of credited future service during the year your disability occurs, or
in at least one of the two previous years. Other requirements

Description for details.
Health & Welfare
At the time of your disability retirement, you may elect to
enroll in the Retiree Health & Welfare Plan if you meet the
eligibility requirements of the Plan. To be eligible you must have
25,000 hours of contributions to the Health & Welfare Fund or
have had health & welfare contributions (that include retiree funding)
for 60 of the past 84 months. Other
requirements must also be met, please review the

details.
When you become eligible for Medicare you
are required to notify the Administrative Office and you will automatically be
moved to a Medicare Eligible retiree health & welfare plan. Be sure you enroll
in Medicare when you are eligible to participate. The health
plan will calculate your benefits based on the amount Medicare would
have paid for treatment whether or not you actually enroll in
Medicare.
Money Purchase Plan (Defined
Contribution)
If you become totally and permanently
disabled, you are entitled to receive the funds in your Money Purchase
account. You must apply for the distribution and elect a form of
payment. To initiate a distribution, please call the
Administrative Office and ask to speak with a Pension Representative.
