Ask for a PPO! Tell Your Providers Where You Want Care

Within the Municipality of Anchorage, avoid paying out-ofnetwork penalties for health care expenses by choosing a Preferred Provider Organization (PPO) for inpatient or outpatient surgery, imaging services, or physical or occupational therapy.

It’s up to you to make sure your provider knows where you want to receive care. Surgery in your future? Discuss these PPO options with your doctor:

■ Alaska Regional Hospital
■ Mat-Su Regional Medical Center
■ Surgery Center of Anchorage
■ Alaska Surgery Center
■ Alpine Surgery Center

Having a lab test done? Ask your provider to send blood, urine, culture, biopsy, or other samples to:

■ Alaska Regional Hospital
■ Mat-Su Regional Medical Center

Doctor ordered an imaging test? For X-rays, CT scans, MRIs, ultrasounds, mammograms, or any other imaging tests, tell your provider you want to go to:

■ Alaska Regional Hospital
■ Mat-Su Regional Medical Center

The Out-of-Network Penalty Adds Up…Fast

Within the Municipality of Anchorage, the Plan will apply the following penalties if you use a nonPPO provider for services available at a PPO:

■ The amount the Plan allows for covered charges will be limited to the PPO rate or 50% of the covered charge.
■ Then the percentage the Plan pays (of the allowed amount) will be reduced by 20%.
■ For non-emergency inpatient hospital stays, you’ll also pay a $1,000 penalty.
■ You will be responsible for the billed amount that exceeds the Plan’s allowed amount.
